Required evidence
Each required item needs files attached, a note identifying equivalent evidence, or a not-applicable note.
Include balance sheet, income statement, statement type for each year, and an audited statement within the last two years when available.
Include recurring owner assessment or maintenance-fee income, structural reserve contributions, and general reserve contributions.
Include stated purpose, outstanding balance, and available commitment when known.
Upload the latest report if one is required for the property, or note why the requirement does not apply.
Include preparer, study date, review level, site-work date, 25-year funding plan or cash-flow detail, and assumptions.
Use the formal study if maintained; otherwise upload the board-approved reserve plan, budget reserve schedule, or comparable planning support.
Upload ACORD forms when available, or attach comparable broker or carrier evidence that confirms limits and coverage.
The appraisal or estimate supports insurance adequacy and should include the date and appraised insurable value.
Include declaration, bylaws, articles of incorporation, rules and regulations, and current amendments.
Supporting evidence
Fire/life-safety, elevator, permit, local authority, DBPR, or similar records when applicable.
Property/windstorm, flood if applicable, general liability, D&O/E&O, fidelity/crime, and umbrella/excess if maintained.
Broker/carrier summaries, claim logs, or board materials showing claim year, type, status, amount paid/incurred, and legal-dispute status.
Approved minutes from the most recent annual meeting and board meetings held during the past 12 months.
Upload the current contract if the association uses professional management.
Officer role, election or appointment date, term expiration, years served, certification/training status, and known vacancies or turnover.
Investment of funds, disbursement approvals, vendor insurance, contractor closeout, lien releases, or comparable controls.
Written program, responsible party, and last-updated date when maintained.
Engineering, project-management, legal, or other professional oversight for significant capital or reserve-funded projects.
Documentation for separate catastrophe funding, cash fund, letter of credit, or related emergency reserve.
Pre-event restoration vendor agreement or emergency response terms, if maintained.
